MID-YEAR REPORT


Mid-Year reports are required for all LSTA funded projects. The purpose of the report is to provide the Division with a status report of project activities and expenditures to assist staff with project monitoring. Reports are divided into two sections:
  1. Expenditures - from both grant and matching funds to date

  2. Narrative - a description of project activities and results to date

Expenditures
The expenditure page should show actual expenditures to date for grant related activities by both category of expenditure and source of funds - grant and matching.

The third column is for unexpended project funds.

Narrative
The narrative should consist of a detailed, descriptive report of the project in sufficient detail to make its purposes, activities and results to date clear. It should discuss the progress made toward meeting the project's outcomes, any successes, problems or setbacks which have affected attainment of the outcomes established in the original proposal or revision.

Signatures
The library administrative head (director of system or cooperative) must sign the cover page of the report.

Deadline
Reports are due on April 1st.
