Following the proposal review process, applicants are advised of their grant awards for the applicable fiscal year. The "Notification of Grant Award" form is used for this purpose. This form is the only official notification of grant award and should be maintained as a part of the official project files. On the form, each project to be funded is listed along with the award amount, Catalog of Federal Domestic Assistance (CFDA) number, and any special project related comments or instructions. The form is also used to notify grant recipients of any modification (increases or decreases) in the award amount. The CFDA number for LSTA grants is: 45.310 Sample Notification of Grant Award form |
